Answerphone facilities are available after hours. Medical Questionnaire: It is helpful and time-saving if a medical/lifestyle questionnaire can be completed prior to your first appointment. When possible we prefer to e-mail this directly to you; alternatively, please arrive 15 minutes early to allow time to complete the form.
